Job description
Job purpose:
* To contribute to the operational delivery of an effective Vulnerable People Service as part of a team responsible for safeguarding, promoting the welfare of and continuously improving outcomes for vulnerable people.
* To manage a complex caseload, delivering high quality assessments and support plans that address the needs of vulnerable people through the commissioning of individual packages of care, whilst managing the reputation of the Council.
Responsibilities include, but are not limited to:
* Holding and effectively managing a complex and varied caseload with appropriate supervision, guidance and support, reflecting the function of the team and the Vulnerable People Service in accordance with policy, procedures, guidance and legislation.
* Undertaking work within legislative frameworks including the preparation of written statements and representations to the judiciary at Court hearings.
* Undertaking direct work with adults and their families in line with care plans.
* Working co-operatively with both internal and external colleagues across multi - agency boundaries.
* Attending and representing the department at a range of multi-disciplinary forums and undertaking the role of chair as required.
* Taking responsibility for obtaining regular professional supervision to ensure effective practice, reflection and career development.
* Ensuring that reports are up to date, of a high quality and submitted according to appropriate timescales.
* Undertaking any other duties as directed from time-to-time to meet the exigencies of the service.
Requirements:
* Social Work Degree.
* Registration with HCPC.
* Minimum 2 years post qualification experience in statutory Adult’s Social Care service.
* Knowledge of NHS and Community Care Act, Mental Health Act other appropriate legislative frameworks, statutory guidance and processes for vulnerable adults.
* Ability to apply critical reflection and analysis to complex cases.
* Ability to demonstrate a commitment to own professional development and that of other colleagues.
* Ability to understand and demonstrate a commitment to equality and diversity.
* Enhanced DBS dated within the last 12 months. On the update service? You could be fast tracked through our recruitment process today.
* Willingness to complete an enhanced DBS if you do not currently hold one.
