This is a new and exciting service for Norfolk Community Health and Care. As a Community Intravenous Nurse you will be delivering a bespoke and intricate service in peoples own dwellings, allowing them to continue hospital prescribed treatment at home, thus shortening their time in hospital.We are looking for dynamic and patient focused nurses who want to advance their clinical skills.
You will liaise directly with team members across the acute and community services and be responsible for monitoring all aspects of the patients response to their treatment and feeding back to the acute trust when required and seeking support from the clinical lead when needed.Extensive training and support will be provided once in post, however previous IV experience desirable., Your work with the community IV team will involve visiting patients in their own homes and occasionally nursing homes/community inpatient settings. You will be responsible for managing your own caseload, visiting patients at their scheduled time, assessing/maintaining their vascular access device, sampling and monitoring blood reports and safe delivery of the prescribed IV therapy. You will also be responsible for replacing IV access (within your competency skill-set) and performing routine investigations such as observations/NEWS scoring.
You will be expected to discuss any concerns with the IV clinical lead, clear lines of escalation will give you easy access to clinical advice and a management plan as and when needed.
