The East IHC Team is seeking to recruit two dynamic, motivational, forward thinking Primary Care Cluster Leads in North West Flintshire and North West Wrexham. The successful candidates will play a crucial role in working with with Area management, Collaborative Leads and Cluster team in leading the development of Accelerated Cluster Development (ACD). Main duties of the job The post holder will work across the health and social care partnership (primary care, community health and social care), and will be highly motivated and a skilled communicator. The post holder will lead the planning, evaluation and delivery of local services according to population need and will promote the development of effective, efficient primary care and community services for their cluster. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
