Dementia Link Worker
Part-time: 22.2 hours per week
Salary: SP8 £23,915 – SP11 £25,166 per annum pro rata (dependent on experience & qualifications)
Permanent subject to funding

PURPOSE OF JOB
SOAR Community are seeking to recruit a Dementia Link Worker who will act as the single point of access for referrals from Sheffield Specialist Dementia Advice Service and partner organisations. Working alongside our Dementia Peer Support Worker to ensure people living with dementia and their carer or family, are better informed about the options available to support their dementia journey in the community. The role will:
• Complete an initial ‘triage and assessment’ for people living with dementia and their carer by telephone or virtual means
• Ensure clients and their carer requiring tailored support are booked into the Dementia Peer Support Worker calendar for further assessment, including making arrangements for an initial home visit
• Undertake 6 Monthly dementia wellbeing check in calls to ascertain change in condition(s), additional support or caring needs
• Support the Dementia Peer Support Worker with groups/events when required
• Maintain relationships with local VCSE community organisations, Primary Care Networks, Dementia Specialist Advice Service and the PKW Community partnerships.
