DUTIES AND RESPONSIBILITIES Provide administrative support to the CNSs. Book and outcome telephone clinic appointments. Maintain the patient Database and ensure all information is up to date and accurate. Order supplies and patient information as requested by the CNSs. Assist the CNSs with the management of the patient pathway, ensuring appointments have been requested and booked. Organise and prioritise the designated workload to meet the needs of the service. Patient Care Transition Assist the CNS with handover of patient care to other teams for safe and effective transition of care. Communicate effectively with patients and health and social disciplines, using various methods, such as electronic or in person. Provide a single point of access, including rapid re-entry into the system for those with urgent or specialist needs. Triage telephone and email enquiries using a risk assessment framework. Patient Engagement Ensure patients/carers know how to contact the service when their condition/needs change, including out of hours. Provide general information and support about cancer services using Trust approved literature. Sign-post patients and relatives/carers to appropriate services such as Maggies and Macmillan. Care Delivery Demonstrate ability to recognize situations requiring rapid response from a healthcare professional and respond appropriately. Evaluate outcomes of care delivery with the registered practitioner. Act as an advocate and facilitator for patients; link with CNSs to resolve issues. Ensure accurate documentation of patient care in accordance with hospital policies and standards. Assist with the rollout of Holistic Needs Assessment and electronic patient outcomes (eproms). Professional/Clinical Leadership Act as a role model to colleagues and be an effective resource to patients, carers, and colleagues. Support clinical developments and projects to promote excellence in patient care. Assist in evaluation and audit of such developments. Attend and participate in CNS Away Days and Strategy Meetings. Recognize personal limitations and work within the job description parameters, seeking help appropriately. Education and Development Participate in annual appraisal and accept personal responsibility for growth and development within the role. Ensure compliance with Trust essential training. Attend educational meetings and conferences to keep updated with current best practice. Develop information and resources for patients and their carers with other professional colleagues. Research and Audit Assist CNSs in audit and evaluation of service delivery and help identify areas for improvement and change.
