Seeking to ensure that every young person has an opportunity to develop fully in body, mind and spirit we operate large supported housing hostels, children’s centres, community health clubs, community hubs, early years nurseries and a wide range of specialist youth services and clubs. Altogether, this equates to our team of around 400 amazing colleagues annually delivering over £15 million of high impact and valuable community services.

Whilst part of a rapidly expanding staff team, currently with over 100 employees, this role is part of a team of 3 specialist workers delivering accommodation and support to Single Homeless Individuals, as part of the local Homelessness Prevention Pathway, in line with the organisations overarching mission of ending homelessness and rough sleeping. Helping each resident to progress towards independence and independent living.
Support will be delivered intensively on a person centric and individual needs basis, utilising a dynamic and innovative multi-agency, and positive value based culture and shared goals approach.
Responsibilities
• Have an understanding of complex needs and therapeutic approaches to support the residents in addressing their needs and inform change
• Have an in-depth understanding of childhood trauma and its impact on mental wellbeing and be able to deliver the appropriate information, intervention, advice guidance.
• Provide day to day support to a caseload of residents with complex and coexisting needs, ensuring they are appropriately assessed and supported and to work collaboratively with specialist substance misuse services and Adult Mental Health Services to ensure these needs are met.
• To ensure each resident has a clear programme of support in place to help them achieve the independent living skills required for them to progress onto a more or even completely, independent living placement / accommodation.
• You work closely with all partner agencies to ensure a coordinated multiagency response to all substance use issues, in particular specialist substance misuse services and CMHT.
• To ensure the health and safety of the residents; in supporting them to maintain and further improve their mental health, and to promote their independent living abilities in accordance with their individual support plans, fully involving them in all aspects of the planning and service delivery.
• Undertake referral assessments to determine the suitability of accommodation and levels of support needs.
• Complete all appropriated documentation in relation to incoming residents, including risk assessments, occupancy agreement, benefit related applications.
• To develop and maintain operational links with key partners and stakeholders keeping them fully informed of the pathways and support needs of the residents.
