Job summary
We are seeking a highly motivated individual with excellent interpersonal and communication skills to work within the Ashton, Leigh and Wigan Community Cardiology Service. They must be committed to supporting and working with patients with a diagnosis of myocardial infarction/coronary revascularisation as part of the Long Term Conditions management. The successful candidate will join a Consultant Led Multidisciplinary team, committed to partnership working within community services.
This is a hours permanent post.
Service hours are Monday to Friday 0800-1700.
Applicants should already be working as a cardiac nurse or have significant cardiology experience at Band 6 or above, with a genuine interest in cardiac rehabilitation or long term conditions management. Non-medical prescribing is preferred but will be expected to undertake within 12 months.
An Enhanced DBS with Adults' Barred List checks will be obtained during pre-employment checks.
This post involves contact with patients and families face to face within a community setting.

Main duties of the job
The Community Cardiac Rehabilitation service provides individual, evidence-based care for patients with a confirmed diagnosis of Myocardial Infarction/Revascularisation to the patients who have a GP within the boroughs of Ashton, Wigan and Leigh. The purpose of the service is to educate, support, and improve the quality of life of those diagnosed with a heart attack/unstable angina and to prevent unnecessary hospital admissions working collaboratively with key stakeholders.
The job holder in partnership with members of the multidisciplinary team will be expected to provide evidence-based care and provide expert advice for patients and their carers with complex cardiac conditions, providing relevant treatments as required.
The post holder will lead on clinical audit activity within their specialist area where they will contribute / lead the ongoing development of clinical practice, standards of care within the service including the development of policies, procedures, protocols, and guidelines.
The post holder is accountable for their safe, effective clinical practice and working as part of a defined team and supporting team performance, ensuring efficient and effective use of physical and human resources.

Key results from the job holder
1. Be professionally accountable for the maintenance of standards and professional practice as set by Trust Policies/Procedures, the NMC code of Professional standards of Practice and Behaviour and relevant professional guidelines.
2. Be an experienced practitioner who is confident in managing a caseload of post myocardial infarction/revascularisation patients in a variety of settings, working autonomously within local and national guidelines in achieving and maintaining regional and national targets throughout the patients care episode.
3. As an independent prescriber, prescribe evidence-based medicines within appropriate scope of practice adhering to the Trusts prescribing policy and guidelines.
4. Provide specialist nursing advice to support individualised care to patients and relatives/carers without supervision and provide peer to peer support for the wider teams within primary, secondary and community services.
5. Undertake a variety of physical observations appropriate to the scope of practice using a range of equipment, record and report and escalate exceptions as required to relevant multi-disciplinary team members.
6. To ensure that the patient pathways are in place to increase the referrals to the wider cardiac rehabilitation team (Occupational therapy and Physiotherapy) and to the heart failure nursing team as appropriate.
7. Provide mentorship and leadership support for all staff who are developing their roles within the immediate and wider teams.
8. Work with the service lead for ongoing development of policies and protocols to enhance service delivery in collaboration with cross organisational partnerships.
9. Maintain and update personal clinical expertise in accordance with their professional regulatory body and contribute as a role model to uphold the delivery of exceptional nursing care utilising reflection as a tool for learning.
10. In collaboration with the team leader, take the lead on customer care initiatives to monitor the quality of nursing care and action audit information to achieve agreed standards set within the service specifications and within the Trusts policies and procedures.
Planning and Organisational Duties
11. To implement specialist advice and demonstrate delivery of autonomous evidence-based treatments with the aim of improving the quality of care to the target population to enhance their quality of life and prevent unnecessary hospital admissions.
12. Assess and evaluate inbound referrals to be able to accept or decline according to the service specification and notify the correct health care professional of the decision.
13. Clinically assess, plan, implement and evaluate patients individual plan of care by providing specialist advice and education within the patients own homes or community clinics, taking appropriate action in accordance with national/local policies and protocols for the treatment and care of patients post myocardial infarction/revascularisation.
14. Manage individual patient caseload and support those of the wider team to safely discharge patients as per the local policy.
15. Ensure that electronic or written documentation is maintained and recorded within the professional registration requirements and the Trusts standards utilising regular audits to measure compliance.
16. To implement and adhere to the Trusts framework for Clinical Governance within their area of practice.
17. Identify clinical risk and act according to the trusts Risk Management policy, gaining advice and reporting as required to maintain the safety of the patient, themselves, and others.
18. To be able to assess the complex health care needs of patients competently with chronic diseases and support the patient/carer in agreeing a holistic individualised plan of care.
19. Carry out venepuncture as and when required in accordance with Trusts policies and protocols.
20. Support patients and their families in managing their symptoms post myocardial infarction.

21. To provide sound clinical leadership and develop support mechanisms for sharing good practice within the clinical setting and within the wider Trust.
22. Be able to confidently work within a multidisciplinary team across a variety of partnerships to develop, implement and evaluate an integrated service through teaching, research, and clinical audit.
23. Participate in organised regular multidisciplinary team meetings, clinics, and patient reviews where the post holder will co-ordinate treatment independently or in conjunction with the wider multidisciplinary team.
24. To maintain a seamless service with key stakeholders so that patients receive the highest possible delivery of care.

Responsibility for Finance
36. To manage the efficient and effective use of supplies, provisions, and equipment within delegated budgets.
37. In conjunction with Line Manager identify, analyse and discuss cost pressures and financial constraints in a timely manner.

Compliance with the Health & Safety at Work Act 1974 the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
Responsibility for Teaching
As a confident specialist nurse, it will be expected that formal and non-formal teaching will be delivered in a variety of formats encompassing existing knowledge to all key stakeholders as required.
Work Circumstances & on-call
The working hours are Monday to Friday between the hours of 8am to 6pm but may change in the future to 7 day working reflecting the changing needs of the service.
