An exciting opportunity has arisen for an enthusiastic and motivated physiotherapist to join our dynamic Admission Avoidance and Discharge Service (AADS) at ELFT. We are proud to be part of a successful partnership between health and social care, driving a proactive approach to care for the residents of Tower Hamlets. This role offers the unique opportunity to work at the forefront of integrated care, collaborating closely with Reablement Service and Initial Assessment Team (IAT) to deliver timely and effective physiotherapy interventions that prevent unnecessary hospital admissions. Moreover, the opportunity to work collaboratively across services, including pioneering work with the Podiatry Service to support homeless populations in Tower Hamlets.
We are looking for a highly motivated, innovative, and adaptable physiotherapist to join our team in working with patients across a variety of clinical areas to deliver proactive care in partnership with multidisciplinary teams. Applicants must have strong communication and collaboration skills and have experience in community care, with commitment to working across health, social care, and voluntary sectors boundaries.
You will need to be a versatile, adaptable team player who is prepared to work across a 7-day rota.

Main duties and responsibilities
* To manage a defined caseload using evidence based/client centred principles to assess, plan, implement and evaluate interventions, maintaining associated records.
* To provide a high standard physiotherapy service to the specialist caseload across various sites within Tower Hamlet as the service requires focusing on admission avoidance and supporting timely hospital discharges.
* Actively participate in the Homeless Project, providing drop-in sessions and delivering health promotion activities to empower homeless individuals to improve their mobility, foot health, and general well-being.
* Engage in holistic care planning for vulnerable populations, addressing barriers to care and promoting self-management strategies.
* To perform advanced physiotherapeutic assessment of patients with diverse presentations and complex physical and psychological conditions to provide a diagnosis and develop and deliver an individualized treatment program.
* To provide leadership for more junior staff including supervision of students on placement, as agreed with senior staff and deputies for the Senior Physiotherapists in their absence as required.
* To participate in CPD and develop skills and knowledge through participation in the in-service training scheme.
* To undertake lone working when required.
* To be able to work flexibly and participate in potential future 7-day working developments (when required).

Person Specification
Education/ Qualification/ Training
Essential
* Diploma/Degree in Physiotherapy.
* Member of HCPC.
* Evidence of CPD maintained portfolio including attendance at recent post graduate courses relevant to this post.
Experience
Essential
* Clinical experience in a broad range of specialties & the ability to work as a key person within the scope of practice in the Multi-disciplinary team.
* Knowledge of a broad range of clinical conditions particularly working with older people and multi-pathologies.
* Experience working in a client centred care, within a multidisciplinary team.
* Experience in conducting appraisal & supervision for junior members of staff.
* Extensive experience working in community setting in various clinical areas.
Desirable
* Experience working in musculoskeletal clinics and has completed relevant orthopaedic courses.
* Experience working with Adult Social Care.
Knowledge and Skills
Essential
* Ability to manage own workload, flexibility and use initiative to problem solve.
* Ability in delivering teaching & training to wide range of multidisciplinary team.
* Good interpersonal and strong communication skills.
* Ability to plan, coordinate a range of complex activities in relation to clinical and operational work.
* Awareness of potential hazards including frequent verbal aggression and risk of physical aggression in settings such as the clients home, community venues, public transport etc.), whilst ensuring safety of self, client and members of the public.
* Ability to assist clients who are frail or who requires assistance with activities of daily living and to apply a Rehabilitation approach in a generic Health care professional role.
* Information Technology skills e.g. word-processing/excel etc.
* An understanding of the current changes within the NHS.
* An understanding of the needs of Tower Hamlet's culturally diverse population.
* Understanding of Health and Safety issues in relation to working in a client's home.
* Knowledge of statutory and voluntary agencies that may be involved in supporting older people at home.
