We are looking for an enthusiastic and adaptable Community Nurse to join our caring and progressive team in Dover.
You will work as part of a close-knit team dedicated to the health and wellbeing of people in Dover. Dover is made up of a diverse patient group with complex needs.
District nurses make a difference by ensuring our services are proactive and patient-focused, helping to keep people at home and independent for longer.
Our community nursing teams are closely aligned with local Primary Care Networks to ensure patient care is holistic and joined up. We believe in treating the whole person, not just the condition, so we take time to get to know our patients.
Main duties of the job
Community nurses provide complex nursing care to people at home and in residential care homes.
As a community nurse, you'll:
* Assess the healthcare needs of patients and families
* Monitor the quality of care they're receiving
* Be professionally accountable for its delivery
Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from hospital, be terminally ill, or have physical disabilities.
As a Community Nurse, you will work as part of a team to keep people safe and well at home. You will use your clinical judgment to support evidence-based care planning for people living at home, in care homes, or other settings and ensure that people who use our services have a good experience.

One way we support this is by being a flexible employer. This service operates seven days a week from 8 am - 10 pm, allowing us to support flexible working hours. Weekend working is generally on a 1:4 ratio with days off during the week.

Person Specification
Qualifications
* Registered Healthcare Professional with relevant Professional body (1st Level Nurse/ NMC)
* Degree in a health relevant subject or willingness to complete a degree (within 3 years of appointment)
Experience
* Experience of recent community/acute healthcare gained through education or work experience placement or previous employment
* Experience of providing treatments in patients' own home/care home
Knowledge
* Awareness of professional issues and recent developments in the NHS
* Knowledge of up-to-date clinical practice
Skills and Abilities
* Sound communication and interpersonal skills (verbal and written)
* Ability to organize own work
* Ability to cope with change
* Ability to use own initiative
* Able to drive with a full UK Driving Licence
