The post will involve home visits and school visits so ability to travel is essential. The post is part time 16 hours per week. You must be currently registered with the NMC as a nurse associate. Main duties of the job This post will be part of a team who care for children with Diabetes in the community. The postholder will work closely with hospital, community and primary care services. The key purpose of the role is deliver high quality targeted family support to families of children diagnosed with diabetes. The post holder's responsibilities will include- supporting families with adjustment to diagnosis, providing parenting support, strengthening family relationships, advising on benefits that are available such as financial advice, support and access to other services. The post holder will be responsible for developing a directory of local services to be used as a resource by the team and the families. They will also be responsible for delivering group interventions for parents and young people. The support will be provided in the home, community and hospital settings. The post holder will work collaboratively with the family, diabetes team and any professional or voluntary services supporting the family and will be required to develop their knowledge about diabetes and how it ismanaged in children.
