We are seeking a Nurse to support the Welwyn and Hatfield Adult Disability Team on a part time, permanent basis. You will be working within a health and social care setting and supporting health partners such as GPs and other health professionals to improve the health of people with disabilities. Main duties of the job You will develop relationships with wider health partners to address the health inequalities for people with learning disabilities identified through the LeDeR Reports and increase the uptake of health initiatives by engaging people with learning disabilities and their carers. You will have access to a range of training and on the job training to enable you to develop as a practitioner and take part in projects with partners such as NHSE. About us Applicants must have knowledge of the requirements and challenges of community nursing and be able to demonstrate transferrable skills from other nursing environments if you have not had community experience. You must be registered as a Nurse with a valid NMC registration and if your registration is not in Learning Disability Nursing be able to demonstrate experience, skills and knowledge in working in this area. We are based in Stevenage however we encourage smart working with the benefit of a laptop and smart phone. You would be required to be office based 40% of the week and you have the option of working at home or at one of our touchdowns across the area.
