Job overview
We have an exciting opportunity for an enthusiastic and dedicated NMC Registered Community Nurse to join our Rapid Response Nursing team.
Our service operates 24 hours, 7 days a week to offer nursing interventions to patients living in their own homes. We work in an integrated way with health and care providers to ensure that care is joined up. We strive to care for acutely unwell patients at home, helping them to maintain independence and ensuring that high quality care is delivered in an efficient and holistic manner.
You will play an integral role in the development of the service as we are working innovatively to continually improve our outcomes for patients and our experience for staff in delivering care.
This post would be suitable if you are a newly qualified nurse or have years of experience. We offer an extensive and supportive preceptorship programme with enhanced support for nurses working within our community environment to ensure you receive the best support into your community nursing career.
Due to the nature of this role, you will be required to travel independently across a large geographical area and ideally hold a valid driving licence and have access to a car for work purposes.
You will be expected to work on a rota basis covering 24/7 shift patterns, but we will, where possible, offer flexible working options.
Main duties of the job
As a community nurse you will be able to demonstrate your passion and ability to assess, plan, provide and evaluate evidence-based nursing care for patients experiencing a wide range of conditions.
As part of this role, you will be expected to:
* Support our patients and work with people in an asset-based way that really enables them to live their optimum life, being self-caring and independent with their health needs where possible.
* Work collaboratively, efficiently and creatively with multi-professional colleagues and partners with a focus on helping patients to avoid hospital admissions and where they do need a hospital stay, facilitating timely and well-supported discharge back home.
* Manage referrals received from professional clinicians, e.g. ambulance services, GP's, Hospital staff.
* Provide IV antibiotics, line care, wound care, IM/SC injections, Peg Feeds, Venepuncture, Cannulation, Palliative care, as well as all general nursing skills.
* Future development to work towards physical assessment qualification.

Because we are an innovative, integrated Community service covering the East Staffordshire area including the Burton Town area, Uttoxeter and the surrounding Villages, we work closely across the Nursing teams, AHP’s including Podiatry, Physio, Falls Team, Stroke and Rehab team including our out of hours service, to ensure that our patients are at the centre of the care provided. We collaboratively deliver our care across all the professions, recognising the importance of a patient-centred approach, working together across all the nursing and therapy teams.

Person specification
Qualifications
* Degree in nursing or equivalent
* Registration with NMC
* Working towards Mentorship qualification (998-997)
Experience
* Relevant experience/placements to the role including assessment and management of long-term conditions, palliative care conditions and wound care
* Ability to manage time and caseload effectively
* Supporting service users, families and carers in dealing with people with nursing needs
* Working in a multidisciplinary team
* Experience in the delivery of healthcare
* Relevant placements including surgical, medical, palliative care
You must have appropriate UK professional registration.
