Triage and Musculoskeletal Assessment, employing shared decision making and high level clinical reasoning to formulate care plans, provide treatment, appropriately refer onward and signpost patients to appropriate resources.
Working as part of a large Integrated Musculoskeletal Service Team, alongside teams such as the Orthopedic, Rheumatology, Outpatient Physiotherapy, Surgical Services, First Contact Practitioners and Community Pain Management to provide safe, efficient and effective patient care, whilst continuously striving to promote Service Improvement.
To be part of a highly motivated team, providing a high standard of clinical care within the iMSK team specialising in provision of foot and ankle pathology management.
Evaluation of the effectiveness of the iMSK service via audit and research.
Continued practice of leadership skills in line with organisational values, with direct line of sight to the iMSK Vision.
To identify, develop and adhere to policies, strategies and objectives of the iMSK team and employing NHS Trust.
Main duties of the job
Undertake the advanced assessment and treatment of musculoskeletal patients (including some rheumatological and persistent pain conditions) and patients presenting with low back pain and radicular pain (sciatica) in primary and secondary care as part of the iMSK service within the ESP or First Contact Practitioner role, who may have complex and/or chronic presentation and to determine clinical diagnosis and formulate individualised management and treatment plans. This may include advice and discharge, referral to other professions for management, requesting further investigations and diagnostic procedures (MRI, X-rays, Ultrasound, and blood tests) and interpretation of the results or referral to Specialist Consultant. Practitioners require specialist knowledge of multiple disciplines (rehabilitation, surgery, pain management) in order to appropriately execute the assessment and support and inform on the direction of the patient’s care pathway.
To work in an extended role in the management of patients with diverse presentations and complex physical, medical and psychological conditions. To make a clinical diagnosis, by using advanced assessments, requesting and interpreting the results of advanced diagnostic tests and investigations, to implement the correct care pathway. Ensuring referral to the appropriate services and / or facilitating Choice to service users for Secondary Care, independently of medical staff.

Detailed job description and main responsibilities
The post holder will be required to develop effective communication and good working relationships with all members of the multidisciplinary team, outside agencies, families and carers.

To provide clinical and professional advice and to develop, promote and evaluate collaborative approaches to an innovative clinical practice both internally and externally.
To provide highly specialised and expert advice to MSK, Physiotherapy or Podiatry colleagues and other members of the multidisciplinary team.
To listen, empathise, communicate and reassure effectively, using appropriate tools, to fully explore management options with shared decision making, including cases with distressful diagnoses.
The post holder will demonstrate understanding of complex facts or situations requiring analysis, interpretation, and comparison of a range of options. Will also undertake analysis and interpretation of results of medical investigations such as MRI where judgements regarding a range of clinical issues and/or highly complex patient conditions will have to be made (Radiology reports alone are not adequate to make these judgements)
To triage referrals using skills beyond the scope of podiatry to aid diagnosis and development of treatment plans.
To lead the specialist area in Audit, Research & Development in collaboration with the General Manager and Service lead, Therapy Consultants, Clinical Team Lead, and other members of the Musculoskeletal Team.
To undertake complex musculoskeletal assessment and specialised treatment on a daily basis. This may require periods of concentration and fine motor skills.
To maintain accurate, comprehensive and up-to-date documentation in line with legal and departmental requirements, and to demonstrate a reduction in the Consultant Orthopaedic Surgeons waiting list for patients with a neuro musculoskeletal disorder.
