Participating in the delivery of individual treatment plans across boundaries of care, working closely with senior staff and Care Coordinators providing support and care according to PCN team Model and My Care and Support plans. Main duties of the job To provide support and give time to service users within Navigo who are difficult to engage and have complex needs. To provide holistic care, taking into account psychosocial and physical healthcare needs of the client group and to undertake rehabilitation activities alongside service users.
