Job Overview: We are looking for a skilled Band 6 Occupational Therapist to support patients in an acute medicine setting. This role involves specialist assessment, collaborative discharge planning, and contributing to patients' smooth transition to ongoing care. Key Responsibilities: Conduct comprehensive assessments to identify patients' needs in acute medicine. Develop and implement effective discharge strategies with patients, families, and the multidisciplinary team. Provide expert advice on rehabilitation and ongoing care plans. Maintain accurate and timely documentation in line with Trust standards. Participate in team meetings, case reviews, and service improvement initiatives. Requirements: HCPC registration as an Occupational Therapist. Experience in acute medicine, particularly in assessment and discharge planning. Strong clinical reasoning and decision-making skills. Excellent communication and teamwork abilities. Dedication to delivering patient-centered, high-quality care.
