Job Description
To participate in the activities of an Integrated Community Learning Disability Team receiving requests for and undertaking holistic assessments of client’s needs and commissioning packages of care working across professional boundaries when required.
Requirements
1. Co-ordinate the assessment of individual needs and prepare care plans to meet said needs
2. Secure the resources or services required in responding to the identified needs
3. Develop relationships with service providers/other professionals
The Individual
1. Experience of working in a Health/Social Care setting
2. Demonstrable understanding of main issues facing Community Care
3. SSSC Registered
4. Effective communication skills (written and oral)
5. Ability to manage change
6. Ability to work to deadlines and under pressure
7. Self-confidence
8. Self-motivated
9. Ability to manage own workload
10. Competent use of IT equipment, software and good keyboard skills
11. Effective assessment and organisational skills
12. Finance skills
13. Ability to negotiate and liaise in relation to care management
14. You will be expected to travel efficiently and effectively between various work locations within Moray to meet the operational requirements of the Service. Due to the rural nature of Moray this is normally undertaken by use of a car/van.
