Job summary
Harpenden Health Primary Care Network (PCN) are seeking an enthusiastic and proactive care coordinator, on a part time basis for 18-22 hours per week, with flexibility on the times worked due to personal circumstances.
Main duties of the job
Supporting a cluster of three practices, the successful candidate will be liaising verbally with patients who have complex health needs and members of the local voluntary sector.
1. Supporting the clinical team in coordinating all key activity, including access to services, advice, and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
2. Supporting digital initiatives.
3. Responsibility for the co-ordination of the patients' journeys through primary care.
