Main duties of the job
As a Specialist Community Public Health Nurse, you will have excellent communication skills and a broad knowledge of working in the community with children and their wider family.
You will be a solution focused team player who will support the delivery of the universal offer in a 0-19 integrated neighbourhood team.
You will manage a universal plus and partnership plus caseload, working collaboratively with other agencies to ensure protection and the best health outcomes for children and families in Bolton. As a Specialist Community Public Health Nurse you will work within the community dealing with clients from diverse backgrounds with varying levels of need.
Specialist Community Public Health Nurses in Bolton deliver an evidence based service using a wide range of assessment and intervention tools as well as providing excellent support alongside a wider multi-disciplinary 0-19 service.

SCPHN qualification in School Nursing is essential to apply for this role only!
Specialist community public health school nurses work as autonomous practitioners in Bolton and as part of the integrated 0-19/25 neighbourhood teams and lead on the delivery of The Healthy Child Programme, providing every child with a schedule of health and development reviews, health promotion, screening, and support for parents, carers, children and young people tailored to their needs. Universal health and development reviews are key features of the programme and aim to assess children, young people and families strengths, needs and risks and provide parents, cares, children and young people with the opportunity to discuss their concerns and aspirations. Growth and development are assessed at key points with the aim of promoting health and wellbeing and the early detection and management of health needs. Additional support is provided at key times to protect children, young people and families who are experiencing harm or are at risk of suffering significant harm. It supports a model of progressive universalism a core universal programme for all children with additional contacts for those with identified need.
