Description Are you a compassionate and skilled Registered Nurse looking to make a meaningful impact in community care? MannaCare is seeking a Community Registered Nurse to provide clinical nursing services within our Community Programs, including HCP, CHSP, and NDIS within the Manningham Council area. This role is crucial in ensuring the health and well-being of clients by delivering high-quality, person-centred care. Community Registered Nurse will play a key role in assessing, planning, and reviewing client care needs. You will collaborate with health professionals and service providers to ensure clients receive the best care while maintaining their independence at home. Key Responsibilities: Provide clinical nursing services within the Community Programs. Conduct comprehensive client assessments, develop care plans, and ensure ongoing monitoring. Utilise evidence-based practice to make sound clinical decisions. Collaborate with key stakeholders, including health professionals and community organisations. Support clients in achieving maximum independence while respecting their personal, emotional, psychological, cultural, and spiritual needs. Act as an advocate for clients and ensure they receive equitable and inclusive services. Participate in continuous improvement initiatives and quality monitoring. Maintain accurate clinical documentation to ensure compliance with industry standards. What We Are Looking For: Registered Nurse with current AHPRA registration (mandatory). Experience in a community nursing or similar role. Strong knowledge of aged care, disability services, and community health frameworks. Ability to conduct clinical risk assessments, care planning, and care coordination. Strong interpersonal and communication skills, with a commitment to person-centred care. Proficiency in clinical IT systems and electronic documentation. Current NDIS Check and Driver’s Licence. Awareness of cultural and linguistic diversity in service delivery. Why Join Us?
