We are looking for a Pharmacy Technician to join our established pharmacy team, working across 4 GP practices to develop and manage medicines optimisation. This will involve working on a range of projects to deliver the best possible prescribing outcomes across the PCN.

Main duties of the job
Primary care is a fast-changing and innovative environment where priorities can change as the demand of our patients change. In general terms, the main responsibilities for the post holder are:
1. Supporting practices with medication reconciliation and repeat prescribing processes, working towards undertaking a patient interactive role to ensure effective medicines use through shared decision-making conversations with patients.
2. Supporting the practices to achieve cost-effective prescribing.
3. Providing advice and support as part of the MDT; to promote the clinically appropriate use of medicines at practice level.
4. Developing protocols to support long-term condition management and safer prescribing.

Job responsibilities
Job Purpose
To work with the clinicians in each practice team to rationalise and manage medicines, prescribing, repeat prescribing and the medicine review processes within each practice.
To support practices in the delivery of local and national NHS / ICB targets and objectives.
Management Line
Clinical management will be through the Clinical Pharmacist lead with line management provided by the Belper PCN management team.
Knowledge, Skills And Experience
* Professional registration with GPhC
* Evidence of continued professional development
* Demonstrate ability to influence partners and stakeholders of the respective merits of different medication management options
* Awareness of systems to support management of patients in a primary care setting
* Excellent clinical pharmacy knowledge including terminology
* Understanding of pharmacy law, ethics and current legislation
* Self-motivated, able to manage own time and work on own initiative
* Understand and be able to work in a multidisciplinary team
* Clear understanding of patient confidentiality framework

Key Working Areas
Primary Roles:
1. Support the practices to achieve cost-effective prescribing.
2. Provide advice and support as part of the MDT; to promote the clinically appropriate use of medicines at practice level.
3. Carry out medicines optimisation tasks under the direction of PCN Pharmacists.
4. Work towards undertaking a patient-facing role to ensure effective medicines use through shared decision-making conversations with patients.
5. Day to Day Workload to include:
* Direct discussions with patients on a range of medicine-related issues e.g. medication compliance and post-discharge reviews.
* Support pharmacists in medication reviews / deprescribing for care home patients.
* Medicines reconciliation after hospital discharge and from clinic letters.
* Manage shared protocols and liaise with pharmacist for more complex patients.
* Support clinical pharmacist in structured medication reviews.
* Support initiatives to reduce inappropriate antibiotic prescribing.
* Carry out audits on prescribing as required.
* Provide training and support to reception and administration teams.
