We have a fantastic opportunity for a part time band 5 nurse looking to move into the field of Community LD Nursing to join our Community Intervention Service supporting people with mental health issues in the community to lead fulfilling lives based within the Enfield Integrated Learning Disability Service. The post is full time, but part time hours and job share may be considered.
The Team
Enfield Integrated Learning Disability Service is a multidisciplinary service consisting of community nurses, social workers, transition specialists, physiotherapists, occupational therapists, speech and language therapists, Equals Employment workers, art therapists, psychologist and psychiatrists.
The works in partnership with other agencies across the borough to support people with a learning disability with their physical and mental health needs.
The position will provide the opportunity to develop skills in specialist Learning Disability nursing, mentoring students, pathway working and multi-disciplinary work whilst being part of a supportive team.
We will provide regular clinical supervision and you will be supported to continue your professional development.
Main duties of the job
To work as an integral member of the Community Nursing Team for People with Learning Disabilities. To assess the care needs, design, develop and implement and evaluate programmes of care in partnership with clients and carers. To provide clinical advice/support to people providing services to the client group as a member of the wider Integrated Learning Disabilities Service. To assist in developing services. To liaise appropriately with other agencies. To maintain a designated caseload. Participate in teaching staff/students, where appropriate.
To positively represent the needs and rights of PLD, tacking health inequalities and improving access to NHS services for PLD through facilitating a close working relationship between primary and specialist services, Voluntary and Private Organisations, PLD and Carers.

Knowledge and Experience

16. 1 year post registration experience in a Learning Disability Community or Inpatient area or equivalent
17. Knowledge of relevant legislation to support those with a learning disability to live independent lives
18. Knowledge and experience in the application of the Mental Health Act
19. Knowledge and experience in the application of the Mental Capacity Act
20. Experience of implementing Positive behavior support plans
Skills and Abilities
Essential
21. Ability to complete nursing and risk assessments for people with a learning disability and their families
22. Ability to assess and develop plans of care for a person with a learning disability
