Job overview
North CMHT is looking for an experienced band 6 CPN or other registered mental health professional to support the team in the functions of duty and assessments.
Within the context of Transformation of Community Mental Health and Wellbeing services within Birmingham, you will be supported to give input and shape the service to be effective and responsive for those in need, delivering high-quality care.
Main duties of the job
The post holder:
1. Will support duty and assessment functions within the CMHT.
2. Will support in delivering a responsive duty service for the CMHT population.
3. Will act as an independent practitioner within a defined community setting, while liaising with other professionals and agencies as necessary.
4. Will be responsible for the triage of new referrals with support from the clinical lead, completion of new patient assessments, and care planning within agreed parameters.
5. Will be working closely with Neighbourhood Mental Health Team colleagues in line with transformation to help ensure individuals are seen by the most appropriate professionals in a timely way.
6. Will be supported in creatively approaching challenges, with a focus on reducing waiting times and improving service user experience.
7. Will have a working knowledge of local and national policies and legislation, which govern current service provision and audit.
8. Will be responsible for ensuring the promotion of equality, diversity, and rights in accordance with good practice and legislation.
9. Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence-based care as appropriate.
