What the Job Involves Undertake assessments of Adults and Carers in line with Care Act 2014 and local procedures. Develop individual support plans reflecting assessed needs. Promote Direct Payments to offer choice and control. Provide practical support and information to Adults and Carers. Monitor and review support plans and services' effectiveness. Engage with users and carers consistently with anti-discriminatory practices. Collaborate with team colleagues and professionals to meet service user needs. Maintain records following departmental policy. Help colleagues with preliminary safeguarding enquiries. Participate in duty operations and respond accordingly. Contribute to service development by providing information and future service ideas. Work across teams within the Division when needed. Progression to BR9 involves maintaining a higher knowledge level and handling complex cases. Participate in the induction program for new staff and collaborate on complex care pathways. Deal with challenging family dynamics and independently assess care for residential care cases.
