CYC Adult Social Care are looking to recruit one Social Care Worker to join the Hospital Social Work Team (HSWT); this is an unqualified position.
The role requires you to work within a fast-paced environment, within a supportive Hospital Social Work Team facilitating timely and safe hospital discharges. The team works to facilitate rapid hospital discharge and complete assessments post-discharge in line with the Discharge to Assess Guidance. Regular supervision and training are provided. You will hold your own caseload within a supportive team of qualified and unqualified workers.

We recognise that social care can be a challenging job and actively invest in supporting our social work teams.
If you are interested in applying for this role, you will need a working knowledge of legislation relating to Adult Social Care, Safeguarding Adults, and Mental Capacity. You will have excellent communication skills as well as skills in assessment, partnership working, and accountable practice.

As this role is public-facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C1: Can express themselves fluently and spontaneously, almost effortlessly. Only a conceptually difficult subject can hinder a natural, smooth flow of language.
Hybrid working arrangements are available for this role. Each role at CYC is different, and your working arrangements will be determined based on the requirements of the role and in collaboration with you and your needs as part of the recruitment process. Your contractual location will be the designated council office for your team.
