Deliver child-centred, holistic family assessments using a range of evidence-based tools to identify adverse childhood experiences and factors impacting family functioning. The goal is to increase resilience, enhance parenting capacity and promote secure child-parent attachment to support positive child outcomes.
Respond resourcefully to families at risk of poor outcomes at the earliest opportunity, making full use of community resources and settings to support children’s development and reduce family isolation, ensuring outcomes are sustained.

2. Promote a safe environment that safeguards and protects children, ensuring that all safeguarding procedures are adhered to.
3. Provide structured, individual support to children and families using appropriate assessment tools, such as Hackney’s Early Help Assessment Form, and create individual plans with agreed outcomes, targets and review dates.
4. Act as lead professional for individual families, coordinating and sharing information at MAT meetings and supporting and leading Team Around the Child meetings.
5. Support parents/carers in their care of their child through targeted home-based work, group work and the coordination of services and activities. This includes providing support with barriers such as housing and financial matters that impact parenting.
6. Coordinate and deliver parenting groups that help parents/carers develop a deeper understanding of their child’s needs and strengthen their parenting skills.
7. Work closely with partner agencies to deliver cohesive, consistent support tailored to the individual needs of each family.
8. Maintain accurate, up-to-date records and reports in line with organisational and statutory requirements.
Essential Criteria:
1. Minimum Level 3 qualification in childcare and education, primary education, work with parents, health or social work.
2. Proven experience in working directly with children and families, providing early help and intensive support.
