to join Pen-Y-Bont surgery.
This AdvancedNursePractitioner role will involve working alongside a multidisciplinaryteam, including 4 GP Partners, a Prescribing Nurse, Practice Nurse and Healthcare assistant.
The successful ANP will work as an autonomous practitioner,diagnosing, managing, prescribing and discharging patients with illness orinjury across all age groups including young children. This may be byface-to-face consultation, telephone or video consultation.
Main duties of the job
The successful post-holder willmanage a caseload and deal with a wide range of health needs in a primary caresetting, ensuring the highest standards of care for all registered andtemporary patients.
The successful applicant will undertake a variety of duties, including surgery consultations, telephoneconsultations and queries, visiting patients at home( if community ANPs unable to attend), checking and signing repeatprescriptions, dealing with queries, and referrals.
KeyResponsibilities:
Ten minute telephone consultations and Fifteen minute face to face consultations
Share of home visits.
Able to manage acutepresentations
Able to manage chronicconditions
Able to order appropriateinvestigations (bloods/radiology).
Able to refer appropriately tosecondary care or organise emergency admissions.
Share of admin: clinical letterswhich require actioning/filing.
Able to file pathology results
Able to see all age groups
No restrictions on prescribing.
Able to see pregnant women inregards non pregnancy related issues.
Able to deal with mental healthpresentations
Hours - Part-time / negiotable. This can be confirmed during the interview.

The postholder will demonstrate critical thinking, is a caring, compassionate and committedexperienced nurse who, acting within their professional boundaries, willprovide care for the presenting patient from initial history taking, clinicalassessment, diagnosis, treatment and evaluation of care. They will demonstratesafe, competent clinical decision-making and expert care, including assessmentand diagnostic skills, for patients within the general practice. The postholder will commit to demonstrating critical thinking in the safe clinicaldecision-making process. They will communicate and work collaboratively withthe general practice team to meet the needs of patients, supporting thedelivery of policy and procedures, and providing nurse leadership as required.

* Be an independent prescriber
