Job Purpose To work effectively as part of a team to lead and supervise Rehabilitation Support Workers (RSW). The postholder will line manage junior members of staff, including reporting sickness, completing appraisals and supervision and discussing with their supervisor any performance management issues. The post holder will also be expected to complete clinical duties face to face with patients, to include completing initial assessments, personal care calls and rehab visits. Support with reporting and audits within the service as required. Support the management with ensuring the RSW training and competencies are up to date. Taking active role in daily MDT and becoming the link between the RSW team and the clinical team. To work with registered therapists regarding patient care and take on case management duties to ensure patients rehabilitation goals are being completed and patients being discharged in a timely manner. Monitor and ensure all predicted discharge dates are up-to-date and patients are discharged appropriately. Supporting the clinical team in completing referrals to social services and reviewing predicted discharge dates. To ensure that the patient experience is to a high standard in line with CityCare values. Develop protocols for own work and propose policy or service changes in own area. To play a key role in the development of an innovative service providing excellent discharge practices. Providing training and support around discharge excellence. Liaise with all members of multidisciplinary teams to ensure effective multidirectional communication in order to deliver a high quality service to the patient and their relatives. To challenge decisions appropriately to ensure the best outcome is achieved for the Patient. Act as a liaison point with the clinicians to ensure escalation of issues that affect service delivery. The focal point of contact for up to date information in relation to discharge communication, particularly for patients and carers, providing good quality up-to-date relevant discharge information. To maintain a focus on continuity of care and the patient experience at all times.
