An exciting job role has recently become available at the Rotherham Metropolitan Borough Council. They are looking for a new member for their Hospital Discharge Team.
What will your responsibilities be?
As a Social Worker, you will facilitate safe discharges for individuals from hospital back into the community. You will build on individuals’ strengths, assets, and abilities to maximize their independence and promote self-management whenever possible. Additionally, you will collaborate closely with health colleagues and other partners to help individuals meet their outcomes and lead fulfilling lives.

Qualifications and Experiences:
The ideal candidate will have a strong understanding of safeguarding practices and the ability to identify areas of need and risk of harm.
