Nurse Consultant for Adult Palliative Care

An excellent opportunity has arisen for a Nurse Consultant for Adult Palliative Care. This role will be part of the Growing the Community and Friarage collaborative team working in close collaboration with the Head of Nursing, Lead Nurse for SPC and EOLC, Clinical Director and Service Manager. You will provide strong and effective clinical leadership to the specialist palliative care nursing team across the community and acute services. You will act as an expert practitioner providing advice and consultancy for the management of patients with complex specialist palliative care needs. You will work with patients and staff, evaluating existing services and providing innovative and novel models of care. You will act as an exemplary role model for expert clinical nursing practice, demonstrating high professional standards, values and attitudes with a patient-centred approach.

Main Duties of the Job
This role will be at an advanced clinical level in direct delivery of high-quality palliative care making critical clinical decisions based on expert knowledge and clinical assessment skills, working across the four pillars of advanced clinical practice as set out by Health Education England (2017).
The post holder will provide clinical leadership and support within the nursing/AHP SPC teams in developing the service to improve the patient experience, supporting Palliative Medicine Consultants with day-to-day clinical activity.
The postholder will provide expert clinical care to patients with specialist palliative care needs via direct assessment and also by providing expert knowledge and assistance to clinical teams across the trust. To provide the highest standard of expert holistic care for patients through utilisation of advanced clinical skills and advice in the assessment and management of patients, working autonomously and with the multi-disciplinary team to improve the quality and safety of patient care.

Experience
* Significant experience working within the specialist palliative care environment
* Experience of assessment, diagnosis and care planning within a complex care setting
* Experience of working autonomously
* Experience of effectively managing a team and resources
* Experience of innovation in practice
* Experience of change management and service improvement
* Experience of leading audits and implementing these changes in practice
Qualification and Training
* Registered Adult Nurse with current NMC registration
* Advanced Communication Skills training
* Masters degree relevant to professional nursing practice
* Post graduate certificate (level 7) in a relevant field
* Physical Assessment and Disease Management Skills Course
* Evidence of ongoing professional development
* Access to a vehicle for work purposes to meet the requirements of the role

* Teaching Qualification or equivalent
* Sage and Thyme Facilitator qualification
* Advanced Communication Facilitator training
