Job Summary
Llety Newydd is a complex care unit, which provides long-term care to patients who still require hospital provision.
The team comprises of a unit manager, deputy, registered nurses and health care support workers. In addition to this the team will work alongside the inpatient multi-disciplinary team to work towards discharge to the community.
The ability to speak Welsh is desirable but not essential for this post.
Main Duties of the Job
The ideal candidate will have excellent communication skills, a positive attitude towards supporting people with challenging behaviour and experience of working effectively within a multi-disciplinary environment.
1. Candidates would assist in the provision of both personal and health care, contributing to comprehensive treatment plans for each individual.
2. Candidates would support individuals to access meaningful activities and recognise early warning signs that require care plan intervention.
3. Candidates must also be able to demonstrate commitment to developing their skills.

Job Description
Job Responsibilities
1. You will deliver safe, high quality, compassionate care following individualised care plans and treatment interventions determined by the Registered Practitioner. This will include supporting patients with all aspects of the fundamentals of care including activities of daily living, providing feedback on progress against the plans.
2. You will communicate effectively with patients and relatives in a manner which is sensitive to patients, carers and relatives needs. This includes communicating effectively with patients who may have sensory impairment, speech and language difficulties, memory and cognitive impairment, learning difficulties and challenging behaviour.
3. You will maintain clear and accurate patient records relating to patient care and interventions utilising both digital and paper based records.
4. You will work as an effective member of the team, actively promoting good team relationships.
5. You will take responsibility for attending statutory, mandatory training and work based learning opportunities in accordance with the health board policy and local training needs analysis to achieve compliance with mandatory training.
Person Specification
Essential Qualifications & Knowledge
* Minimum of Qualification Credit Framework - QCF Level 3 (formerly NVQ Level 3)
* Evidence of maintaining their personal development, demonstrates an understanding of the scope of the role in the context of the nursing team
* Evidence of working within specialist care setting
* Knowledge and understanding of the Code of Conduct for Healthcare Support Workers in Wales
* Knowledge of the fundamental's aspects of care and the importance of the promotion of health and wellbeing, understanding of basic physiology, e.g. normal vital signs, fluid balance, nutritional requirements.

* Adhere to and can demonstrate SBU Values & Behaviours
* Caring, compassionate attitude to patients and clients
* Able to communicate effectively with patients/relatives and carers and all members of the multi-disciplinary team.
* Ability to work effectively as a team player under appropriate supervision, and as part of a multidisciplinary team.
* Ability to deal with non-routine and unpredictable nature of the workload, prioritise workload and individual patient contact.
