Nottingham City Care is looking for an experienced, innovative, enthusiastic, and motivated individual to join our clinicians within the virtual ward team in Nottingham City.
The Virtual ward supports patients, who would otherwise be in hospital, to receive the acute care, remote monitoring, and treatment they need in their own home or usual place of residence.
Our Virtual Ward Matrons are an integral part of our virtual ward team providing leadership, clinical expertise, and knowledge to our patients and fellow clinicians. The aim is to reduce hospital admissions and facilitate safe early discharges to reduce the amount of acute hospital bed days.
The Virtual ward provides expert clinical care and support for patients who have an acute illness, by offering additional support from a multi-disciplinary team, under the direction of a medical consultant (Step down) or NEMS GP (step up), to enable them to be treated in their own homes. The patients will remain on the virtual ward, under the care of a medical consultant/GP for up to 14 days.
The core hours of the virtual ward team are 8am to 8pm, 7 days a week and the clinical team work a variety of shifts across the week and an average of 1 weekend in 4.
You will be part of the wider multi-disciplinary team, working in collaboration with consultants, senior nurses, assistant practitioners, GPs, physiotherapists, OTs, pharmacists, secondary care, and social services.
Main duties of the job
You will:
* Provide strong clinical leadership to the VW team.
* Be an Independent prescriber and either hold the V300 qualification or be ready to undertake the course within 12 months of commencing in post.
* Carry out physical examinations.
* Provide intense clinical input and monitoring to acutely unwell patients.
* Use your expert knowledge and clinical judgment to identify the potential diagnosis and where appropriate make a final diagnosis.
* Use your extensive practice experience to plan and provide skilled and competent care that meets patients' health and social care needs, involving other members of the healthcare team as appropriate.
* Ensure the provision of continuity of care, including follow-up visits.
* Assess and evaluate with patients, the effectiveness of the treatment and care provided and make changes as needed.
* Work independently, although often as part of a healthcare team.
* See patients in their own home and ensure that each patient's treatment and care is based on best practice.
* Work as part of multi-professional and multi-agency teams.
* Provide supervision, education, and support to patients and colleagues as appropriate.
* Support the Clinical Service Manager in operational management of the team.

Job responsibilities
Job Purpose:
Virtual wards support patients, who would otherwise be in hospital, to receive the acute care, remote monitoring, and treatment they need in their own home or usual place of residence. This will consist of a mixed caseload of the step down from hospital whereby patients are still under the care of a hospital consultant and step up from the community where the medical oversight is provided by NEMS GPs.
Virtual wards provide acute clinical care at home for a short duration (up to 14 days) as an alternative to care in hospital. The overriding purpose of the Virtual Ward Matron is to support the provision of the highest quality patient care to individuals whilst on the virtual ward pathway, supporting the service manager.
The post holder will support the virtual ward registered staff and clinical team ensuring the delivery of high-quality patient care upholding patient dignity delivered with kindness and compassion. Clearly demonstrating a clinical leadership role, with responsibility for planning, coordinating, and delivering care as appropriate per the daily MDT, initiating and managing medical and nursing care interventions, coordinating the care of people with highly complex needs, leading care and coordinating partner agencies and stakeholders. The successful candidate will hold the NMP qualification or be ready to undertake this within 12 months of being in post.
The Virtual Ward Team core hours are 08.00 to 20.00 over 7 days, covered by a shift pattern of 08.00-16.00, 10.00-18.00, and 12.00-20.00 also working an average of 1 weekend in 4. Each clinician will be expected to be part of the Rota.
Role Context:
* To provide appropriate and effective care management through the virtual ward pathway unnecessary hospital admission from the Community managing the active caseload and providing necessary oversight and support.

Qualifications
* Level 1 Registered General Nurse or registered AHP.
