Job summary The Community Mental Health Assessment Team offers a screening and assessment service to adults (aged under 65) in the Bradford District and Craven region. Those referred to our team are those felt to be potentially experiencing complex or severe mental health difficulties and who may need specialist support. The team is comprised of Community Mental Health Nurses, Social Workers, a Nurse Prescriber, and Psychological Therapists. We are closely linked with Community Mental Health Teams in the Bradford District and Craven region. When mental health needs are identified that may need support from one of these teams, we are able to plan and transfer care to them. We carefully consider each referral. Where appropriate we are able to give advice to Primary Care or help people to access other forms of support available in the local community. The Community Mental Health Assessment Team works alongside all CMHTs in the Trust with a need to work flexibly at sites, home visits as well as home working. The post holder will provide high standards of skilled mental health care and assessment to individuals and their families. The role will include active participation in the service delivery and development alongside multi-disciplinary teams. The Community Mental Health Nurse will be an active member of the multi disciplinary team and will work in close co-operation with secondary and primary care based professionals, other community agencies and voluntary and third-sector services.
