Main duties of the job
This is ahands-on patient facing role; we are seeking individuals to join our team whoare caring, compassionate and confident, who demonstrate effectivecommunication, excellent customer service skills, good IT skills and theability to support patients with interventions and/or queries. The postholderwill be a key member of the practice team, responsible for undertaking patienthealth checks inc. BP / ECG monitoring / height and weight, taking bloodsamples, health and wellbeing advice and promotion.

1. Over 15 years of experience of delivering community outpatient services
2. Contracts with ICBs and hospital providers
3. A diverse clinical workforce of consultants, GPs with extended roles, specialist nurses and allied health professionals
4. Delivery of over 120,000 patient consultations per year
5. Delivery of 1st consultations within a maximum of 4 weeks from referral from a choice of community locations, 7 days per week
