Social Prescriber Link Worker- MAPLE PCN
Salary: From £26,500 WTE dependant on experience.
Working hours: 3 days a week max 22.5 hours per week with the flexibility of additional hours.
33 days annual leave inclusive of bank holidays.
An exciting opportunity has arisen for a Social Prescriber to join our well-established team at MAPLE.

MAPLE Primary Care Network delivers services to over 42,500 registered patients across 3 Practices: Oakfield Surgery, Mandeville Surgery, and Poplar Grove Practice.
What is Social Prescribing?
Social prescribing is about giving people time to focus on what matters to them and taking a holistic approach to health and wellbeing. It connects people to community groups and statutory services for practical and emotional support.
Main duties of the job
Social prescribing can help strengthen community resilience and reduce health inequalities by addressing the wider determinants of health, such as debt, poor housing, and physical inactivity. It particularly benefits people with long-term conditions, those who are lonely or isolated, or have complex social needs affecting their wellbeing.
You'll be working with individuals facing long-term health conditions, loneliness, or complex social needs. Each day will be varied, allowing you to engage with people of all ages and backgrounds.
What experience will you need?
* Understanding of social prescribing or a person-centred approach.
* Ability to be a good listener and committed to supporting local communities.
* Preferably experience working positively with individuals facing complex social and emotional challenges.
* Great interpersonal skills and a proactive approach to supporting people, community groups, and local organisations.
* Local residency in the community.

Job responsibilities
* Provide personalised, holistic support to individuals, families, and carers to enhance their wellbeing and health outcomes.
* Promote social prescribing to patients and its role in improving health outcomes and reducing social isolation.
* Build relationships with key staff in GP practices within the local PCN and participate in relevant meetings.
* Work in partnership with local agencies to raise awareness of social prescribing.
* Provide regular updates to referrers about social prescribing outcomes.
* Seek feedback about the quality and impact of social prescribing.
* Collaborate with the Prevention Matters service to maximize social prescribing availability.
* Encourage feedback from patients, families, and carers about the impact of social prescribing.
Person Specification
Qualifications
* NVQ Level 3, Advanced level, or equivalent qualifications or working towards.
* Evidence of recent and relevant Continuing Professional Development.
* Training in motivational coaching and interviewing or equivalent experience.
