Job summary An exciting opportunity has arisen for an innovative, enthusiastic and motivated neurology Nurse to join our experienced and friendly Community Neuro Rehabilitation Team, which includes an integrated Stroke Early Supported Discharge (ESD) service. You will be based in the heart of Richmond; a beautiful part of London. There will be a high proportion of home visits. You will also see outpatients in our bright, modern premises, which comprises a supportive reception team and a base to liaise with the rest of the team and upload patient records and other admin tasks. You will provide a nursing service to Stroke patients on discharge from hospital, including medication reviews and secondary prevention education, information and advice. You will receive excellent support from the wider team which comprises Neuro therapists. You will also develop skills in Multiple Sclerosis and Parkinson's by working closely with, and supporting, the experienced specialist nursing team. No weekend or on call work. Free parking. Main duties of the job You will have clinical responsibility for the care and management of patients following a Stroke and work closely with family and carers. You will also support the specialist nursing team to provide support and reviews for people with Multiple Sclerosis and Parkinson's. You will see patients in their own homes, in our outpatient clinic and/or at other appropriate settings to facilitate rehabilitation. You will be part of a dynamic MDT consisting of neuro-specialist Therapists and Nursing.You will participate in delivering education groups for people with Stroke, PD and MS, and secondary prevention advice for people following Stroke. You will be involved in reviewing medication for patients at home and following hospital discharge. You will take the lead in ensuring the wider team is up to date with training in taking observations in line with NEWS2. The whole team contributes to ongoing clinical development and improvement projects to ensure that we provide an excellent service for our patients.
