Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. You will contribute to the effectiveness and efficiencies of our community nursing teams by using your skills and nursing experience tosupport the team with triage of calls and referrals made to the community nursing teams. You will actively liaise with patients; carers and service providers to support clinical decision making and make appropriate decisions following comprehensive remote triage (using available information from referral information and electronic clinical systems) to ensure the safe management, and allocation visits where needed, in response to calls and referrals received by the community nursing service. You are activity encouraged to develop skills via our CPD programmes, and you will be well supported by your peers and team leaders to do this. Due to the nature of community working, ability to travel across the patch to provide care to patients is essential. Main duties of the job As a Band 5 Community Triage Nurse, you will have professional and clinical knowledge of community nursing to provide remote triage of calls and referrals made to the community nursing teams in Burntwood, Lichfield & Tamworth. As Part of this role, you will be expected to: Understand the scope of the community nursing service and the role of other MPFT and external services within each Primary Care Network. To ensure referrals are directed to the most appropriate service (including directing to emergency services where needed). Occasional delivery of care to patients in a community setting in order to maintain up to date clinical experience. Whilst not providing regular face to face patient contact the post holder will need to maintain an up-to-date knowledge of the clinical aspects of community nursing to resolve clinical queries over the phone and to enable effective remote assessment and clinical triage. This includes palliative care (particularly symptom management and support to families in the last few days of life); wound care; medication management; diabetes management; catheter and bowel care. Have supervisory responsibilities in the absence of the triage lead nurse; including coordinating the team; allocating work to team members; resolving and escalating issues; coordinating the daily huddle and group chat and representing the triage team on the community nursing capacity calls. Occasional supervision of students and new nurses joining the community nursing team.
