Job summary
The Virtually Assessed Pathway clinic (VAP) is an award winning service, unique to Velindre Cancer Centre, This post holder will be joining a small team of nurses and pharmacy technicians led by independent prescribers. The VAP Service assesses Patients who are due chemotherapy or SACT in the following 2 - 3 days, these patients are clinically assessed over the telephone and their blood results analysed allowing the SACT to be authorised.
The VAP service is not just a pre SACT assessment clinic, we manage the patients' cancer pathway and as a result we become part of their journey. We maintain continuity of care and as a result we develop a relationship with our patients as we often assess them before each treatment.
The VAP clinic currently works with the following teams assessing patients on suitable regimens from the following cancer sites
Colorectal / Breast / Urology / Gynae / Skin.
The post holder will be responsible for the assessment of care needs, the development, implementation, and evaluation of programmes of care, participating and contributing to the wider clinical governance agenda including service developments and flexible working to meet the needs of patients.
The working week hours can be flexible. This role can be part time / full time/ job share and we can consider compressed weekly hours.

Main duties of the job
1. To work with patients and their carers to determine in partnershipthe assessment of patient needs. Working collaboratively with themulti-disciplinary/multi agency team.
2. To liaise and provide guidance on your area of practice to inpatientand outpatient on the care of patients when requested.
3. To act as the named nurse/ keyworker for designated individuals
4. To ensure that patients receive care according to plan,delegating the care to be given to other members of the team asappropriate.

Qualifications and/ or knowledge
Essential
5. 1st or 2nd Level registered nurse or practitioner.
6. Degree or Working towards a degree.
7. NMC standards.
8. Chemotherapy awareness

9. Teaching and Assessment
10. Clinical Assessment skills. Educated to Degree Level
11. Willing to undertake specialist oncology module in chemotherapy.
12. Understanding of the wider policy influences on the NHS.
Experience
