To support the delivery of the Healthy Child Programme (HCP) 0-19 (DH, 2010), thereby improving outcomes for children and young people and supporting the Public Health agenda, meeting Public Health targets and reducing health inequalities in partnership with key agencies.
Promote the protection of children, adhering to the Local Safeguarding Children's Partnership policies and procedures. Work under the supervision of a band 6 Health Visitor/School Nurse within a designated area, alongside partner agencies such as Action for Children, The Children's Society, Family Hubs, Children's Services, and education systems.
Work as part of a team to ensure that Key Performance Indicator (KPI) targets for the service are met, support service development, standard setting, clinical audit, and benchmarking, delivering a high-quality, need-led, safe and effective service to all client groups.
Main Duties of the Job
* Participate in the assessment of health needs and formulation of care plans for children, young people, their families, and their communities. This includes implementing programmes of care and offering support and advice to parents and families in various settings.
* In collaboration with the public health nursing team, identify individuals and groups within the community where collaborative working and advice would promote public health and healthy living, particularly focusing on disadvantaged groups and addressing health inequality.
* Offer advice and support to children, young people, and adults in accordance with evidence-based practice.
* Actively take responsibility for identifying, accessing, and participating in supervision according to individual needs (e.g., Child protection, CAMHS, managerial).
* Support the delivery of KPIs by working collectively across the area.
* Under the direction of the Specialist Community Public Health Nurse (SCPHN), complete Early Help assessments and work with the multi-agency team to support children, young people, and families in need of additional support and those at risk of significant harm.
About Us
The 0 to 19 Torbay Public Health Nursing team collaborates with Action for Children and the Children's Society to provide preventative and supportive healthcare to children, young people, and their families within Torbay, including monitoring and providing targeted support to vulnerable children.
The post holder works as a member of the 0 to 19 Torbay Public Health Nursing team, maintaining a public health focus under the supervision of a Specialist Community Public Health Nurse to deliver the Healthy Child Programme in various settings.

Qualifications & Knowledge
* NMC registration
* Knowledge of national and local strategies pertaining to young people
* Knowledge of safeguarding procedures
* Experience of working with children and young people
* Previous experience working in a public health role.
