Job summary We are looking for a highly motivated nurse who is experienced in the care of people with chronic lung conditions. They will be responsible for the coordination of care of inpatients at Nottingham University Hospital who have been admitted with an exacerbation of COPD/ILD/bronchiectasis/asthma who are deemed as a respiratory readmission or at high risk of readmission. They will work across respiratory medicine at Nottingham University Hospitals within the COPD nurse specialist team and will also work closely with other respiratory nurse specialist teams and members of the MDT. They will be expected to work independently with confidence as this forms a considerable part of the role. As part of this role visits to the patients home may be required so a full driving license is essential. Attendance at community MDTs will also be expected.
