Assistant Manager - Community Systems and Coordination

About the Role:
The Assistant Manager Community Systems and Coordination will work closely with the Support Coordination Manager and Support Coordinators to provide support with a range of tasks, becoming a valuable interface between operational social care practitioners, hospital discharge teams and service providers. You will assist the Support Coordination Manager to implement a systematic sourcing function which ensures timely access to appropriate services for people living in their community. You will support the team and work in collaboration with our partners in the delivery of services, prioritising urgent care and facilitating the flow of care packages through the system.
About You:
You will have experience of operational issues within a health and social care environment and be proficient in the use of databases, have a sound understanding of contract management and quality assurance processes. With excellent communication skills, you will be able to develop and maintain effective relationships with a wide range of stakeholders. You will need the confidence and ability to demonstrate sound professional judgements when making decisions and have the ability to support real change.
