Community Nursing Sister/Charge Nurse
• Location:Wokingham
• Working Hours:Monday to Friday, 8:30 am to 4:30 pm (part-time options available). Includes one weekend every four weeks, bank holidays on rotation, and one late shift (4:30 pm to 7:00 pm) per week as part of a rota.
• Salary:£39,205 to £47,084
We are seeking an experiencedCommunity Nursing Sister/Charge Nurseto join our Wokingham Community Nursing Services team.

As a compassionate and enthusiastic nurse with excellent interpersonal skills, you will:
• Provide leadership and oversight to the team, supporting and mentoring junior staff.
• Manage a defined caseload of housebound patients with complex nursing needs.
• Ensure the delivery of high-quality, evidence-based care to patients, families, and carers.
• Act as a role model, fostering learning and development within the team.

• Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.
• Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.
• Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients.
• Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.
• Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice.

• Adult nursing qualification along with NMC registration
