An exciting opportunity has arisen for someone to join our therapy team working with patients and their families in the inpatient unit and the day hospice.
* To support the Therapy staff to provide high quality holistic person-centred care to palliative care patients to maximise their quality of life and sense of wellbeing.
* To assist in supporting patients’ individual goals (consistent with their limitations, desires and life plans) and in the maintenance / adaptation of function through the palliative journey.
* To ensure that therapy goals, strategies and procedures are implemented and reinforced throughout the 24-hour period.
* To ensure a seamless service in working flexibly and collaboratively; and communicating effectively with the Therapy team and wider teams.
* To support the Therapy team with clerical and administrative assistance; e.g. maintaining equipment stocks, ordering supplies and managing patient orders.
* To be a point of contact for relatives and carers, ensuring continuity of care within the Therapy and wider teams.
* The post will include sensitively exchanging information with patients and relatives.
