Job summary SBUHB has committed to meet the WHO Elimination of HBV and HCV as a public health threat by 2030. Hepatitis C, proportionally the greatest burden of which in Wales is borne by the residents of Swansea Bay, is a very curable infection with treatment at the earliest opportunity indicated to improve standards of liver health and reduce hospital admission. Application of therapy however is complex due to the social determinants of health, such as homelessness, substance dependence, and incarceration the greatest number of those affected are subject to. Central to the projected work of the BBV Elimination action plan is a core team of dynamic assistant community practitioners who, under the guidance of the specialist senior nursing team and the project manager, will drive successful BBV testing projects across Swansea Bay amongst those most at risk of ongoing transmission of infection to halt the further spread of disease across Swansea Bay. The team will apply a wide range of novel testing methods, adapted to suit the individual needs of the patient, and employ strategies to engage hard to reach adults and a broad range of settings, ensuring equity of care amongst those often excluded from traditional healthcare by virtue of homelessness, substance use, or incarceration. Main duties of the job Tasks and responsibilities will include, but not be limited to: Providing an equitable standard of access to BBV services across the health board, from centres of population such as Swansea city centre to regions with limited access to services such as the further parts of the Afan Valley Tackling the stigma associated with addressing blood-borne virus risk and transmission across diverse communities through education, innovative testing and engagement methods, and providing an excellent standard of visibility and accessibility to the BBV Unit Delivering and driving the thousands of BBV tests annually in SBUHB that are required to identify and treat infection across the region and subsequently and cost-effectively, demonstrate absence of infection to confidently declare elimination Continue to broaden access to locally-innovated and proven methods of collaborative outreach services such as the Wallich Welfare Vehicle mobile clinics, taking care to the patient and delivering a wider range of care by maximising interventions at each interaction Support consultant clinics in outpatients settings to increase the availability of necessary interventions at each interaction, reducing the burden of appointment for each patient by as much as 66% Work under, and develop locally, the principles of Inclusion Health championing and leading access to care for those excluded from traditional healthcare and supporting the training and development of colleagues across SBUHB to do the same About us We believe staff are our best asset and wewant you to be happy and confident about starting your career here in Swansea Bay University Health Board.
