We have an exciting opportunity for a Part-time Practice Nurse to join our Primary Care team at HMP Ford, a category D Prison located in Ford.
This is an exciting time to be part of an integrated and friendly team who are currently planning and embedding the integrated pathway around release and transfer, early days custody and unscheduled care. They currently hold around 350 prisoners but this will increase to 780 when they go through their new build phase, where all teams will be on one site. A similar setting to a GP Practice. Lots of development opportunities.
Hours
Working 28.5 hours a week across 3 days, working 9.5 hour shifts, flexible on what days these are. Working 1 in 3 weekends, Saturday and Sunday - 8-12 noon - 4 hour shifts and when weekend working, you will then only work 2 days during the week.
Salary
This position is part time, you’ll receive an annual salary up to £43,534 per annum FTE - depending on experience.

Candidates are required to have a legal right to work in the UK on a permanent basis and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.
What you'll be doing
As a Practice Nurse, your responsibilities will vary. You will:
* Work together at each stage of the patient journey, supporting them as they arrive into prison (Early Days in Custody Team), delivering the care they need, promoting good health and supporting resettlement (Planned Care Team) and responding to urgent and emergency needs (Unscheduled Care Team).
* You will be accountable for your own practice, including the assessment, diagnosis, treatment and discharge or referral of patients, independent of medical intervention.
* To practice as a Practice Nurse within own scope of clinical competence and agreed acceptable limits of practice. This includes senior leadership in complex wounds, long term conditions and QoF management.
* This multi-disciplinary team is made up of a variety of registered and non-registered professionals including nurses, doctors and mental health/substance misuse practitioners.
* You will deliver face to face care and provide safe and effective health assessments and treatment of patients within the establishment.
* You will also provide assessment, diagnostics, treatment and continued management of patients with acute and long term medical conditions.

Qualifications required
* You need expert clinical skills and demonstrate judgement in assessing and managing wide ranging and complex patient problems.
* Registered Nurse: Adult Nursing with current NMC registration
* Experience of working with long term conditions and working as a Practice Nurse
* Experience working autonomously as a clinical practitioner following best practice guidance and identifying risk to patients through clinical expertise
