Senior Primary Care Dementia Practitioner - Mid locality

An exciting opportunity has arisen in the PCDP service within the Mid locality, where a vacancy is available for a Senior Primary Care Dementia Practitioner. This post will support the existing band 5 PCDPs in the mid locality, offering supervision and guidance and working alongside the leadership team. It will also involve managing a PCDP case list for specific GP Surgeries within Mid Locality.
Main duties of the job
The post holder will be based and embedded within a Primary Care Network (PCN) working with GPs, Dementia Together Service, practice nurses, and other additional roles e.g., social prescribers, health and wellbeing coaches, care coordinators, as part of the primary care Multidisciplinary Team (MDT).
This is a community-based role that requires frequent travel to various locations throughout the county, including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

Person Specification
Education / Qualifications and Relevant Experience
* Substantial experience of managing a busy caseload of clients with Dementia and a range of mental health problems and frailty associated with old age. Experience of working in primary care. Must be working in the PCDP service.
Skills and Aptitude
* Excellent clinical assessment skills.
* Ability to communicate with clarity both verbal and written, and IT skills.
* The use of standardised assessment in people who are frail and complex.
