Caseload Responsibilities:
1. Develop and maintain a caseload of family carers, meeting their complex needs through ongoing assessment and review of health and wellbeing, and the development, implementation, and evaluation of care plans.
2. Advise on, recommend, and provide a range of effective interventions from pre-diagnosis to beyond the death of the person cared for, with attention to loss, transitions, and changing relationships.
3. Promote the acquisition of new skills and competencies for carers to enhance understanding and cope with challenges, promoting good communication among all involved in care.
4. Provide psychological and social interventions to promote health and wellbeing of family carers, assisting with the management of stress, depression, and anxiety related to the caring role.
5. Liaise with GPs, primary health care practitioners, and agencies regarding optimal care for the person with dementia, promoting understanding of carer needs.
6. Promote the health and wellbeing of carers and people with dementia in line with health and social care policy and guidelines.
7. Act as advocates for carers (with permission) to facilitate access to services and influence care provision.
8. Identify development options for addressing inequality and contribute to strategies for improving access to services for socially excluded individuals.
9. Ensure care is delivered according to Admiral Nurse Service Standards and other relevant local and national standards.
10. Organise work under supervision of the clinical lead admiral nurse and collaborate as part of a larger multi-professional team.
11. Recognise the limits of own competency and professional boundaries, making appropriate referrals regarding carer needs.
12. Facilitate and participate in case conferences, care programme approach meetings, and network meetings, providing feedback and liaising with other professionals.
13. Participate in and effectively use clinical supervision opportunities.
14. Ensure nursing practice aligns with NAViGO CIC policies, NMC code of conduct, and that all equipment used is appropriate and effective.
15. Contribute to the development of policies, procedures, and protocols relevant to Admiral nursing and client health and wellbeing.
16. Provide consultancy to others in dementia care and collaborate with health/social services and the voluntary sector as appropriate.
17. Integrate current knowledge of dementia and caring into practice, informed by research and evidence.
18. Participate in practice-based evaluation, audit, and research; disseminate findings.
19. Engage in conferences, seminars, and workshops as agreed with the clinical lead, promoting best practices in dementia care.
20. Promote the rights, interests, needs, and choices of service users in planning, delivering, and evaluating care and services.
21. Facilitate consultation with users about service delivery and provide feedback.
22. Deliver evidence-based dementia care and support improvements in family experiences.
23. Provide support and education to colleagues and other health and social care professionals in dementia care.
24. Support the delivery of key quality and performance indicators related to dementia care.
25. Apply the Admiral Nurse Competency framework at a specialist level, including person-centred care and therapeutic skills.
26. Develop and/or support person-centred care plans for people with dementia, including positive risk management.
27. Ensure compliance with legal requirements, including the Mental Capacity Act 2005, Deprivation of Liberty Safeguards (DoLS) 2007, and Safeguarding Adults 2011.
