Job summary
To assist medical personnel inthe care of practice patients to include treatment, preventative care,screening and patient education.
You will work collaborativelywith the general practice team to meet the needs of patients, following policyand procedures while providing supervision to more junior staff.
Good communications are extremelyimportant within the Practice, with patients and with outside agencies and youwill be expected to help ensure that everyone who comes into contact with itholds the practice in high regard. Absolute discretion is required inprotecting patients confidentiality.
Main duties of the job
Key Responsibilities
1. To work alongside the clinicians in the provision of patient care
2. Monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health care professionals.
3. Undertake annual reviews of patients on relevant chronic disease register.
4. Provide general and specific health screenings to the practice patients (within agreed protocols) and referral to general practitioners as necessary.
5. Advise patients on general health care and minor ailments with referral to GPs as necessary.
6. Following appropriate training, undertake the collection, storage and dispatch of pathological specimens including but not limited to intravenous blood samples, swabs and smears. And ensure the cleanliness of any equipment used. Preform any investigatory procedures requested by the GPs.
7. To recognise changes in the patients recordings which must be reported on and acted on.
8. To work alongside clinicians in emergency management of patients
9. Enable patients to access appropriate professionals in the practice team.
10. Act as a chaperone
11. Clearly present patients points of view to others

We provide care to around 14,000 patients.
Our team consists of 3 GP Partners, 5 salaried GPs, 3 Practice nurses, 2 HCAs, 3 pharmacists, 1 pharmacy tech, Health and Wellbeing team. And we work closely with our PCN.

Primary Responsibilities:
12. Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health
13. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
14. Provide vaccination and travel medicine services
15. Provide wound care (ulcer /Doppler etc.) to patients
16. Request pathology services as necessary
17. Process pathology results as required
18. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
19. Maintain accurate clinical records in conjunction with extant legislation
20. Ensure read codes are used effectively
21. Maintain chronic disease registers
22. Develop, implement, and embed well woman clinics
23. Develop, implement, and embed well man clinics
24. Implement vaccination programmes for adults and children
25. Prioritise health issues and intervene appropriately
26. Recognise, assess, and refer patients presenting with mental health needs
27. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
28. Support the clinical team with all safeguarding matters, in accordance with local and national policies
29. Deliver opportunistic health promotion where appropriate
30. Maintain baby immunisation results logs
