The Pan Dorset Learning Disabilities Intensive Support Team are looking for a highly motivated Skilled Practitioner to Join Our Team.
This is a Full Time 37.5 Hours per week, Band 6 Role, it would be suitable for a Registered Learning Disabilities Nurse/ Mental Health Nurse or Occupational Therapist with a Passion to work along side individuals with a Learning Disability.
You must be a Car driver and have full access to a car this is a community Pan Dorset Post and there is an expectation to support service users across Dorset and at times in Out of area Hospitals.
You should have an interest in working with a trauma informed and person Centred approach. Creativity to engage service user's in a way that is meaningful to them in order to achieve effective out comes for individuals.
A passion to remain up to date with current evidence based practice, to inform all clinical outcomes.
To provide a high-quality person-centred approach to care delivery which always considers people’s safety, privacy, and dignity.
To Provide high Quality Out of area monitoring to maintain local contact and facilitate timely repatriation back to Local area if appropriate.
To provide specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multi-disciplinary care team.
To provide specialist advice to others regarding the management and care of patients / service users.
To devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.
To plan, implement and review health improvement programmes in a range of settings.
Provide supervision, mentorship, training and clinical support to students, trained practitioners, care providers and the wider health and care system.
