What You Will Do:
1. Work collaboratively as a part of a multidisciplinary team to provide comprehensive care to adult patients undergoing a period of inpatient rehabilitation.
2. Implement treatment plans and assist in the timely and safe discharge of patients, ensuring they transition smoothly from inpatient care to independent living or further support services.
3. Make appropriate onward referrals to other healthcare services to support the ongoing needs of patients.
Requirements:
1. Be registered with the HCPC and bring at least 2 years of experience in the physiotherapy specialism.
2. A car driver is preferable to effectively fulfil the needs of this role, as it may require travel between sites.
