Main duties of the job The successful candidate will: hold a strong belief in recovery for people with severe and enduring mental health problems hold a caseload and undertake the role of care coordinator for those with varied complex mental health and social care needs have experience of working in a community based setting will provide direct support and advice to Service Users (Adults of all ages) and Carers, within the recovery framework, embracing the individual right to choice and control ensure that appropriate health and social care services are commissioned as required will also assess, identify and evaluate Service User's needs through comprehensive assessments, reviews, care planning and CPA in conjunction with other professionals.
